What information should be included in my report in the Whistleblower System?

The information contained in your report should be as specific as possible so that it can be properly processed and investigated. Please ensure that the descriptions included in your report can be easily understood by individuals who are not specialists in your field. If possible, include a detailed, chronological description of the facts, addressing the following questions:

  1. What has happened?
    (Provide a clear and specific description of the incident and its context. The more detailed, the better.)
  2. Where did it happen?
    (Production facility, department, etc.)
  3. When did the incident occur?
    (Date or time period and approximate time.)
  4. Who may be responsible for the incident?
    (Name of the person, department, position, or, where applicable, name of the supplier—direct or indirect.)
  5. Who are the individuals or groups affected or harmed?
  6. Which internal policy or regulation has been violated?
  7. What is the estimated cost or value of the damage?
  8. Is there any evidence?
    (Photos, videos, documents, possible witnesses, etc.)
  9. Have you reported the irregularity to anyone else?
    (Direct manager, supervisor, Human Resources, another reporting channel, etc.)
  10. How would you like to be contacted going forward?
    (Provide contact details for communication. Also indicate if you wish to remain anonymous.)
